Registration Forms

This year, we have tried to stream line the registration process.  We have included the waiver as part of the registration form and have allowed for there to be multiple registrants on one form as long as they are in the same household.  If you are registering online through our website on Active.com (Register online), there is a spot for you to initial the waiver.

You can mail in your signed, completed registration and waiver form and check to CancerFree Kids, Attn: Butterfly Walk, Address Pending.  Remember, you can also save time and register online.  If you have any questions, please contact helpdesk@butterflywalk.com

Additional Forms

If you would like to collect pledges by hand in addition to using the Butterfly Walk fundraising website, please use this pledge form.  You may send in your pledge forms and donations at any time to receive credit toward your team's goals.  Mail them to CancerFree Kids, Attn: Butterfly Walk, P.O. Box 575, Loveland, OH 45140.  If you have any questions, please contact helpdesk@butterflywalk.com.
