CAPTAIN'S PLEDGE

In order to become a team captain, you need to provide us with the following information and agree to the pledge noted below. Once you have submitted this form, you will be redirected to the Active website inorder to register yourself/family for the event. Due to a need to have each household sign a liablity waiver, a team captain cannot sign up each of their members. We thank you very much for taking the step to become a team captain.

*		As a team captain, my responsibilities will include the following: I will set a goal to have ten or more people join my team & register themselves/family for the Butterfly Walk; determine a fundraising goal for my team to reach, and encourage team members to seek donations to help achieve that goal. Team Captains have the special ability to establish a team fundraising page on ActiveGiving.com. This is easy to set up and can be used to post a picture and message about your loved one, and encourage team members to use the team page to solicit donations. Any donations the Team Captain receives offline should be turned in on race day.
