Registration Forms We
have streamlined the registration process so that the registration form now includes the event waiver as well. In addition,
you can register up to 6 people in one household on one registration form. You also have this same option if you want
to register online. 2010 mail in registration and waiver form 2010 mail in VIRTUAL registration form You can mail your completed, signed registration form and check
to Cancerfree KIDS, Attn: Butterfly Walk, P.O. Box 575, Loveland, OH 45140. Your registration must be received
in their office by April 23rd. Remember you can still register online until May 5th. If you have any questions, please email helpdesk@butterflywalk.com.
Mail in Donation If
you would like to make a donation by mail, please fill out a pledge coupon and mail it to CancerFree KIDS, Attn: Butterfly Walk, P.O. Box 575, Loveland, OH 45140. If you are sponsoring
a team or team member, please indicate their name on the form.
Fundraising A team captain can create a free team
fundraising website and/or any participate can create a personal fundraising page. See these easy instructions on how to do it. You can also use the main Butterfly Walk fundraising website to collect donations online. If you have collected checks/cash from family and friends, please
turn them in along with this donation form to your team captain or directly to the CancerFree KIDS office. If you have a friend or family member who would like to mail in a donation
directly, you can provide them with a pledge coupon for them to mail in with their donation. Just put your name and
team name along with your team website on the form before you hand them out. At any time you can mail in your pledge
forms to CancerFree KIDS, Attn: Butterfly Walk, P.O. Box 575, Loveland, OH 45140. Remember, everything
needs to be received by May 1st to the CFK office or handed in at the team captains meetings the day before the
event in order for the totals to be counted toward team prizes.
